Families are never just ‘visitors’ to a person with dementia; they are an integral part of that person's life and identity.
We aim to provide excellent care to patients with dementia, and their families during a stay in hospital and community care.
We have the following initiatives in place, in aim of supporting you:
A forget-me-not flower will be placed on the board behind the persons bed, to let staff know that they experience confusion or memory loss, because of a dementia, cognitive impairment, or a delirium. This will allow staff to adapt their communication and care to support your loved one.
As a carer, you are entitled to open visiting times. This allows you to support your loved one, if you so wish, with aspects of care such as company and encouraging with diet and nutrition. To learn more, please speak with the nurse in charge.
Please ask staff for an ‘information about me’ form, which includes personal information about your loved ones likes, dislikes, life history, hobbies, etc, which will aid us in getting to know them.
We have Dementia Champions on our ward. All staff receive specialist training to support those living with dementia, and Dementia Champions receive further specialist training to share with the ward.
We are in the process of recruiting Memory Lane Volunteers, who provide company and stimulation to those living with dementia in hospital.
The hospital has its own Admiral Nurse. Admiral Nurses are specialist dementia nurses who support the whole family. If you wish to be referred, please speak with the nurse in charge.
We run a bi-monthly steering group to improve Dementia care across the Trust. This includes attendance of staff members from each area to contribute to positive change.
