The Medical Examiner Service

The Medical Examiner Service is an independent, statutory service operating in England and Wales which was implemented on the 9th  September 2024. It is a legal requirement that all non-coronial deaths be independently reviewed by a medical examiner.

The service was established to ensure independent oversight, enhance the accuracy of death certificates, and provide additional safeguards for the bereaved. It offers families an opportunity to raise any questions or concerns regarding the care their loved one received.

In cases where a death requires investigation by a coroner after review, the medical examiner is responsible for ensuring that the referral is appropriately made by the attending practitioner.

The Medical Examiners Office has no involvement in direct coroners referrals.
