The Complex Care Service provides support to Children and Young People with complex health needs in East Cheshire.
The service provides clinical care to children and young people with long term complex health needs at home and in the community. Our aim is to optimise quality of life, independence and avoid hospital admissions.
Bespoke Care packages are allocated after a referral has been made, an assessment of needs completed by a health professional, criteria has been met and funding agreed by the CCG.
Who works in the Service
The Team Leader manages the staff and packages of care.
The Nurse Trainer trains and educates the Healthcare Assistants using training tools and competency based assessment.
The Health Care Assistants provide the clinical care and support after completing training and competency assessment.
How to Refer to the Service
Referrals can be completed by Health or Social Care Professionals
